Key Responsibilities:- Provide clinical pharmaceutical care to patients
- Serve as an expert, professional resource to service areas and Pharmacy department
- Work requires strong clinical skills, detailed knowledge of hospital processes/operations, and application of federal and state laws and regulations
- Contribute to Therapeutic Equivalence Policies & Programs
- Recommend Medication Programs & Protocol
- Participate in medication interchanges (IV to PO)
- Participate in Clinical Student programs
- Recommend and monitor clinical adjustments for patient conditions
- Perform Drug Regimen Reviews
- Perform and address Drug Interactions
- Provide Drug Information Services
- Provide General Physician Support
- Recommend and follow current approved therapy regimens
- Provide patient information and education
- Continuously develop expertise in Specialty Units medicine
- Continuously monitor parenteral nutrition programs
- Provide Pharmacy & Hospital Staff orientation and training on medications
- Pharm.D., or residency from an accredited institution
- At least one (1) year of hospital clinical Pharmacy experience
- Preferred: Completion of PGY-1 residency in an acute care setting
- Required: Licensed to practice in the State of Texas
- Required: A.C.P.E. accredited I.V. Certification
- Required: BLS
- Encouraged: Board of Pharmacy Specialty certifications
