Job Title: Pharmacist Clinical Coordinator

Job Summary: ReqOverflow is seeking a highly skilled Pharmacist Clinical Coordinator to join our team. The ideal candidate will have a Doctor of Pharmacy degree from an accredited institution and completion of a PGY-1 residency in an acute care setting with additional 2 years of clinical experience or completion of a PGY-2 residency.

Responsibilities:

  • Provide clinical pharmacy services to patients in an acute care setting.
  • Develop and implement disease state management programs.
  • Collaborate with healthcare teams to improve patient outcomes.
  • Participate in antimicrobial stewardship programs.
  • Maintain ACLS and BLS certifications.

Requirements:

  • Doctor of Pharmacy degree from an accredited institution.
  • Completion of PGY-1 residency in an acute care setting with additional 2 years of clinical experience or completion of PGY-2 residency.
  • Licensed to practice in the State of Texas.
  • ACPE IV certification.
  • Antimicrobial stewardship certification (preferred).
