Position Overview:
As a vital member of our healthcare team, the Phone Triage Nurse plays a crucial role in providing compassionate and expert medical guidance to pediatric patients and their families via telephone. Your contributions will ensure the delivery of high-quality patient care, characterized by empathy and professionalism in every interaction.
Key Responsibilities:
- Manage Patient Communications: Efficiently handle incoming calls, portal messages, and provider communications.
- Conduct Patient Assessments: Utilize established protocols to evaluate patient symptoms and determine appropriate care levels, including emergent, urgent, non-urgent, or home care.
- Provide Medical Guidance: Deliver patient-centered medical advice, instructions, and recommendations.
- Facilitate Referrals: Direct patients to appropriate medical facilities or providers based on established guidelines.
- Document Patient Interactions: Accurately record all patient communications in the EMR system, ensuring comprehensive documentation of assessments and recommendations.
- Prioritize Patient Care: Review patient statuses prior to provider consultations, except in emergencies.
- Evaluate Refill Requests: Confirm the necessity of medication refill requests from pharmacies before forwarding them to the provider.
- Coordinate Medication Refills: Assess patient conditions for antibiotics or asthma medications and suggest appointments as necessary.
- Manage Patient Follow-ups: Oversee recall tasks to ensure continuity of care between patient appointments.
- Provide Backup Support: Assist with ADHD Behavioral and Forms Departments as required.
Qualifications:
A minimum of 1+ years of experience in a similar pediatric role is required.
