Job Title: Foster Care Case Management Specialist

Cornerstones of Care is seeking a dedicated and compassionate Foster Care Case Management Specialist to join our team. As a key member of our professional family support team, you will work closely with youth and parents to connect them with resources and supports, promote resilience, and strengthen families.

Responsibilities:
  • Develop and implement individualized case plans to meet the unique needs of each family
  • Provide emotional support and guidance to youth and parents, promoting healthy relationships and communication
  • Connect families with community resources and services, ensuring access to essential support
  • Collaborate with multidisciplinary teams to ensure comprehensive care and support
  • Monitor and evaluate case progress, making adjustments as needed to achieve optimal outcomes
Qualifications:
  • Bachelor's degree in Social Work, Criminal Justice, Human Services, Education, Counseling, or Psychology
  • At least 2 years of experience in child welfare, with a focus on case management and family support
  • Strong communication and interpersonal skills, with the ability to work effectively with diverse populations
  • Valid driver's license and proof of current vehicle insurance
