Job Title: Clinical Care Specialist - Med Surg

Mass General Brigham Healthcare at Home is seeking a highly skilled Clinical Care Specialist to provide quality, clinical services to patients in various settings in the community.

About the Role:

The successful candidate will be responsible for delivering patient care within organizational philosophy, policy and standards of community health practice. This includes performing admissions process and case management responsibilities in accordance with MGB Healthcare at Home policies and standards of practice, utilizing thorough and timely electronic documentation and interdisciplinary communications.

As a Clinical Care Specialist, you will be expected to provide skilled nursing care in the form of assessment, teaching, treatment, documentation and care coordination, demonstrating high quality customer service and financial awareness.

Responsibilities:
  • Assess patients' needs and develop personalized care plans
  • Provide education and support to patients and their families
  • Collaborate with healthcare teams to coordinate patient care
  • Document patient interactions and outcomes accurately and thoroughly
Qualifications:
  • Bachelor's degree in Nursing or related field
  • Current Massachusetts license as Registered Nurse
  • Minimum 1 year experience in acute care setting
  • Strong interpersonal and customer service skills
  • Ability to work effectively in a team environment
Compensation:

The estimated salary for this role is $83,000 - $118,000 per year, based on location and experience.
