The successful candidate will provide personal care and support to patients in their homes, ensuring their physical, emotional, and spiritual needs are met. Key Responsibilities- Deliver patient care, including personal care, nutrition, and safety
- Assist with daily living activities, such as bathing, dressing, and feeding
- Monitor and report any changes in patient condition to the healthcare team
- Participate in interdisciplinary team conferences and special care conferences
- Document patient information accurately and timely
To be successful in this role, you will need:
- A high school diploma and training/certification in a related field
- Current certification as a Certified Nursing Assistant (CNA)
- Minimum 1 year of experience in home care, preferably working with terminally ill patients
