Job Summary

Waterbury Hospital is seeking a dedicated and organized Patient Referral Coordinator to facilitate the smooth transition of patients from the acute care setting to aftercare facilities, agencies, or community programs.

Key Responsibilities
  • Coordinate patient transitions from hospital to aftercare facilities, community programs, or agencies.
  • Provide information and assistance to internal and external customers regarding aftercare programs and facilities.
  • Manage and organize discharge documents related to patient transitions.
  • Communicate with insurance payers and community providers to facilitate review and approval process for aftercare placement.
  • Maintain accurate records, reports, and statistics related to aftercare placement activities.
Requirements
  • High School Diploma or equivalent.
  • A minimum of three years of experience in a hospital setting or a related health field.
  • Experience in continuing care or home care is preferred.
Skills and Competencies
  • Strong organizational skills with the ability to manage multiple tasks simultaneously.
  • Excellent communication and interpersonal skills to effectively collaborate with patients, families, healthcare providers, and external agencies.
