Job Summary

We are seeking a skilled and compassionate Clinical Care Specialist to join our team at Christian Living Communities. As a Clinical Care Specialist, you will provide high-quality clinical services to our residents, including skilled nursing, assisted living, and all shifts. You will work closely with our care team to coordinate care and services based on resident needs and individual preferences.

Key Responsibilities
  • Provide daily direction and assistance with residents' activities of daily living.
  • Become familiar with residents' preferences to personalize their care.
  • Assist residents with social, personal, and health care needs as required.
  • Ensure knowledge and proper application of safety, infection control, and risk management procedures.
  • Make decisions in emergency situations and notify appropriate personnel.
  • Communicate regularly with nursing supervisors regarding residents' status and condition.
  • Accurately document and promptly report discrepancies to appropriate personnel.
Requirements
  • High School Diploma or equivalent.
  • Certified Nursing Assistant License-active and in good standing.
  • Ability to work well with different teams in different Communities.
  • 3-6 months experience working with senior adults preferred.
  • Excellent customer service skills.
Working Conditions
  • Position requires strenuous physical work, including heavy lifting, pushing, or pulling objects up to 100 pounds occasionally and/or up to 50 pounds frequently.
  • Exposure to blood borne pathogens, body excretions, and communicable diseases possible.
  • Frequent interruptions and interactions with residents, family members, and visitors.
