Role Summary

We are seeking a highly skilled Clinical Excellence Coordinator to join our healthcare team at The University of Vermont Health Network.

About the Role:

  • Deliver high-quality patient care by assessing, planning, implementing, and evaluating the health needs of patients in various states of health and illness.
  • Collaborate with interdisciplinary teams to develop effective care plans that promote healthy outcomes and independence.
  • Maintain accurate and comprehensive records of patient assessments, treatments, and outcomes.
  • Promote patient safety and quality improvement initiatives within the organization.

Requirements:

  • Minimum 1 year of experience as an RN in an ambulatory setting.
  • Demonstrated expertise in clinical assessment, diagnosis, and treatment planning.
  • Strong communication and interpersonal skills, with ability to work effectively with patients, families, and healthcare teams.
  • Ability to maintain confidentiality and handle sensitive information with discretion.
  • Proficiency in electronic medical records and other relevant technologies.

Compensation and Benefits:

  • A competitive salary range of $90,000 - $120,000 per year, depending on experience.

  • A one-time sign-on bonus of $12,000 for external candidates with 1 year of experience.
