Job Summary

WellLife Network is seeking a dedicated and compassionate Direct Support Professional to provide high-quality care and support to individuals with intellectual and developmental disabilities. As a key member of our team, you will play a vital role in enabling our participants to achieve their goals and live independently.

Key Responsibilities
  • Provide personalized support and care to individuals with intellectual and developmental disabilities, promoting their social responsibility, independence, and dignity.
  • Implement individualized plans to maximize each person's level of independence, including budgeting, grooming, nutritional planning, and use of community resources.
  • Ensure the safe operation of agency vehicles and participate in training programs, such as AMAP, SCIP, CPR, and First Aid.
  • Coordinate evacuation procedures and ensure safety in emergency situations, including fire drills.
  • Administer medication and monitor self-administration, documenting progress in the electronic medication administration record system.
  • Respond to crisis situations and incidents, reporting procedures as required.
  • Monitor and record progress on daily checklists, behavior contracts, quarterly summaries, and monthly goal forms.
  • Communicate effectively with colleagues, supervisors, and participants, both verbally and in writing.
  • Participate in routine meetings with participants and management to review goal progress and update program plans.
  • Assist participants in researching and obtaining community resources and participating in recreational activities.
  • Provide training in ADLs and other areas, performing tasks for participants requiring total support.
  • Attend mandatory staff meetings, in-service training sessions, and assigned annual meetings.
  • Maintain confidentiality and adhere to WellLife Network's policies and procedures.
