Job Summary

Alta Mira NM is seeking a highly skilled Early Intervention Service Coordinator to join our team. The ideal candidate will have a strong background in social work, counseling, or a related field, and experience working with families and children with developmental or intellectual disabilities.

Key Responsibilities:

  • Conduct intake and orientation with families, ensuring timely completion of initial CME and ECO reports.
  • Coordinate and facilitate on-going assessments, evaluations, and services, ensuring seamless transitions and effective communication with families and team members.
  • Develop and implement Individualized Family Service Plans (IFSPs), ensuring timely completion and effective delivery of services.
  • Support families in accessing resources and services, including Medicaid and SSI, and provide education on available benefits and programs.
  • Collaborate with team members to ensure comprehensive and coordinated services, and participate in quality assurance procedures to ensure high-quality care.

Requirements:

  • Bachelor's degree in social work, counseling, or a related field.
  • Experience working with families and children with developmental or intellectual disabilities.
  • Strong organizational, communication, and interpersonal skills.
  • Ability to work independently and as part of a team.
