As a Health Coach RN with Blue Cross and Blue Shield Association, you will play a vital role in providing health coaching and case management services to members across the continuum of health.
Key Responsibilities:- Function as a primary nurse for members, providing health coaching and case management services to promote health and wellness.
- Engage with members to develop and support their ability to self-manage their health, including coaching on lifestyle improvement, acute symptoms, and chronic conditions.
- Provide clinical coaching support to members, including conducting surveys and assessments to identify needs and developing care plans.
- Collaborate with members, physicians, and other healthcare professionals to coordinate care and ensure high-quality, cost-effective services are delivered.
- Identify and address barriers to member adherence to evidence-based medicine, and provide education and support to promote positive behavior changes.
- Facilitate communication between patients, families, and healthcare teams, and ensure that members have access to necessary resources and services.
- Current, active, and unrestricted Pennsylvania (PA) Registered Nurse licensure.
- Three years of clinical, case management, and/or disease/condition management experience.
- Professional certification in diabetic education (CDE/CEDES) preferred, but not required.
- Exceptional communication and problem-solving skills.
- Ability to work flexible hours and maintain accurate records.
