Job Summary

Affinia Healthcare seeks an experienced Director of Care Coordination and Support to lead our team of patient support representatives and managed care patient navigators. As a key member of our leadership team, you will be responsible for overseeing the coordination and monitoring of scheduling activities, ensuring efficient daily operations, and driving quality outcomes.

Key responsibilities include supervising and developing our team members, improving communication and collaboration among departments, and ensuring compliance with regulatory matters and quality initiatives. The ideal candidate will have a strong background in clinical settings, excellent leadership skills, and experience with electronic medical records and practice management systems.

Essential Functions
  • Supervise Managed Care Patient Navigators and Patient Support Representatives, tracking attendance, completing performance appraisals, and assisting with interviewing new staff.
  • Responsible for improving communication and collaboration among patient navigators and other departments.
  • Complete periodic audits on patient billing codes per DRVS and portal data provided by managed care plans to confirm appropriate requirements provided by our organization's providers.
  • Develop and implement strategies to improve patient outcomes and quality of care.
