Job Title: Transplant Coordinator Nurse

Description

As a Transplant Coordinator Nurse at Nebraska Medicine, you will play a vital role in providing exceptional patient care and promoting the delivery of high-quality transitional care for patients undergoing solid organ transplants. Your expertise will be instrumental in educating patients and their families about the transplant process, addressing their concerns, and advocating for their needs throughout the care continuum.

Key Responsibilities:

  • Provide comprehensive patient education and support to patients and their families before, during, and after transplant procedures.
  • Collaborate with multidisciplinary teams to ensure seamless care transitions and optimal patient outcomes.
  • Develop and implement individualized care plans that address the unique needs of each patient.
  • Serve as a patient advocate, addressing concerns and resolving issues in a timely and professional manner.
  • Participate in quality improvement initiatives to enhance patient care and outcomes.

Requirements:

  • Minimum of three years of nursing experience, preferably in a medical-surgical or transplant unit.
  • Current Registered Nurse licensure in the state of Nebraska or a compact state.
  • Basic Life Support (BLS) certification.
  • Strong interpersonal and communication skills, with the ability to work effectively with patients, families, and healthcare teams.
  • Ability to work independently and as part of a multidisciplinary team, using good judgment and assessment skills.

Preferred Qualifications:

  • Prior solid organ transplant experience or experience in a related field, such as cardiology or pulmonary care.
  • Master's degree in Nursing (MSN) or a related field.
  • Transplant coordination certification.
