POSITION SUMMARY:
The Home Therapies Registered Nurse (RN) CAP 2 serves as a pivotal role within the Clinical Advancement Program (CAP), catering to newly appointed staff who fulfill the RN CAP 2 criteria or those progressing from RN CAP 1.
This role encompasses accountability for delivering and coordinating clinically proficient care, which includes assessment, planning, intervention, and evaluation for a designated patient group.
Responsibilities may extend to delegating suitable tasks to direct patient care personnel, including but not limited to RNs, LVN/LPNs, and Patient Care Technicians.
As an integral member of the End Stage Kidney Disease (ESKD) healthcare team, this position engages in decision-making, educational initiatives, leadership roles, and quality enhancement activities aimed at improving patient care outcomes and operational efficiency.
KEY RESPONSIBILITIES:
All responsibilities are to be executed in alignment with Fresenius Kidney Care policies, nursing practice standards, and applicable state and federal regulations.
Essential functions are performed under the guidance of the Supervisor, with support from the Educator or Preceptor, or in collaboration with another Registered Nurse.
Conducts ongoing systematic collection and analysis of dialysis data for assigned patients, documenting findings in the patient medical record, making necessary adjustments to treatment plans, and notifying the Supervisor or physician as required.
In collaboration with the patient and home care partner, directs and oversees all aspects of safe and effective dialysis therapy delivery to assigned patients.
Initiates or assists with emergency response protocols.
Acts as a resource, leader, coach, mentor, and role model for both new and existing staff, exemplifying appropriate behaviors, work ethics, and attitudes towards patients, colleagues, supervisors, and the organization.
Identifies expected outcomes, documents, and updates nursing assessments and care plans for assigned patients through collaboration with the Interdisciplinary Team.
Assists in the identification, evaluation, selection, and education of Home Dialysis candidates and their Home Partners.
Evaluates and identifies barriers within the Home Dialysis candidate's environment and assesses the readiness and capability of partners/family to perform dialysis treatments at home.
Trains home dialysis patients and/or Home Partners on the safe and effective operation and maintenance of all Home Dialysis equipment and treatment supplies through a standardized training program.
May serve as a Preceptor for new staff.
Required to fulfill CAP requirements to maintain or advance within the program.
Performs additional duties as assigned by the Supervisor.
WORK ENVIRONMENT AND PHYSICAL DEMANDS:
The physical demands and characteristics of the work environment described here are representative of those encountered while performing the essential functions of this role.
This position involves direct patient care that frequently requires heavy lifting, patient mobility assistance, and ambulation support. Equipment aids and/or colleagues may assist as needed.
Frequent prolonged periods of standing are required, and the employee must be capable of bending over.
Occasionally, the employee may need to move machines and equipment weighing up to 200 lbs. and lift chemical and water solutions weighing up to 30 lbs. to heights of 5 feet.
The work environment is typical of a healthcare facility, featuring controlled air temperature and moderate noise levels.
Exposure to infectious and contagious diseases/materials may occur.
Rotates coverage with other licensed home therapy staff as assigned to ensure reliable and adequate coverage.
This position may require participation in on-call rotations during nights, weekends, holidays, or as defined by program needs.
Travel to training sites, other facilities, and patient homes may be necessary.
May be required to perform essential functions in various locations, including patient homes, with the same physical demands and working conditions as described above.
SUPERVISORY RESPONSIBILITIES:
Oversight of RNs, LPNs/LVNs, Patient Care Technicians, and Home Therapies Care Team Assistants as a Team Leader or designated Nurse in Charge after fulfilling all requirements:
Successful completion of all Fresenius Kidney Care education and training for new employees.
A minimum of 3 months experience in home dialysis therapies as a Registered Nurse is required.
EDUCATION AND LICENSURE:
Graduate of an accredited School of Nursing.
Current appropriate state licensure.
Current or successful completion of CPR/BLS Certification.
Must meet practice requirements in the state of employment.
EXPERIENCE AND REQUIRED SKILLS:
Entry-level for RNs with a minimum of 2 years or more of Nephrology Nursing experience within the last 2 years as a Registered Nurse.
