Key Responsibilities- Articulate the services offered by the organization to patients and their families with empathy
- Employ the nursing process to aid in the development of each patient's comprehensive care plan
- Conduct visits to patients and families to evaluate eligibility and clarify services
- Engage in evening and weekend on-call duties as necessary
- Valid Registered Nurse license in the applicable state
- Bachelor's Degree and C.H.P.N. certification are advantageous
- At least 1 year of general nursing experience required
- Preferred: Minimum of 2 years in Hospice Health Care or Medical Office settings
- Current CPR Certification
