We are seeking a skilled and compassionate Social Worker to join our team at Regency of Fremont. As a Social Worker, you will play a vital role in providing medically-related social services to our residents, helping them to attain and/or maintain their physical, mental, and psychosocial well-being.
Key Responsibilities- Admissions and Social Services Evaluations: Complete thorough assessments of residents' needs and develop personalized care plans to ensure their comfort and well-being.
- Financial and Legal Support: Assist residents with financial and legal matters, such as applying for pensions, referrals to lawyers, and preplanning arrangements with funeral homes.
- Discharge Planning: Coordinate with residents, families, and healthcare providers to ensure safe and successful transitions to other facilities or community-based care.
- Counseling and Psychiatric Services: Provide or arrange for counseling services and psychiatric medication management as needed to support residents' mental health and well-being.
- Documentation and Record-Keeping: Maintain accurate and timely documentation in MDS care plans to ensure compliance with regulatory requirements.
- Current Social Work Licensure: Possess current licensure as a social worker in the state.
- Education and Experience: Hold a Bachelor's degree in social work, with a Master's degree preferred. One year of supervised social work experience in a healthcare setting is also preferred.
