Job Title: HomeCare Registered Nurse Per DIEM

We are seeking a compassionate and dedicated Registered Nurse to join our team in our Monroe office. As a HomeCare Registered Nurse, you will be responsible for providing high-quality care to our adult and geriatric clients on a per visit basis in Senior Living Communities throughout multiple counties in NC, SC Suburban Charlotte area.

Job Summary:

The HomeCare Registered Nurse will be responsible for planning, developing, and directing the programs, services, and employees of the Agency. This includes managing, supervising, coordinating, evaluating, and developing client care teams to ensure quality care delivery and appropriate case management within the Agency scope of services and policies; state, federal, and local laws; and Nurse Practice Act.

Responsibilities:
  • Follow a designated care plan in accordance with patient's needs
  • Make home visits to clients in designated geographic territories
  • Perform assigned duties, including administration of medication, wound care, treatments, and procedures
  • Monitor clients' conditions; reporting changes to Clinical or Client Services Manager
  • Follow up with, execute, and properly document doctors' orders
  • Perform client assessments as necessary
  • Case management and coordination
  • Accurately document observations, interventions, and evaluations pertaining to client care management and services provided
Requirements:
  • A current license as a Registered Nurse in North Carolina
  • Graduation from an accredited and approved nursing program, as indicated by school transcript or diploma
  • Ability to work independently and manage time effectively
  • Strong interpersonal skills
  • Ability to travel to cases as assigned
  • Minimum of two (2) years nursing experience in an acute hospital setting or equivalent experience with one year of supervisory experience with Home Care agency or in Home care clients preferred
Compensation:

$45.00 per hour
