We are seeking a skilled Physical Therapist to join our team at Doctors Subacute Care. As a Physical Therapist, you will play a vital role in helping our residents achieve their rehabilitation goals.
Job Summary:The Physical Therapist will work collaboratively with the therapy team to provide high-quality physical therapy services to our residents. This includes conducting evaluations, creating treatment plans, and providing individualized therapy sessions.
Responsibilities:- Conduct thorough evaluations of residents to identify their physical therapy needs
- Develop and implement individualized treatment plans to meet the unique needs of each resident
- Provide physical therapy services, including exercises, stretches, and other interventions, to help residents achieve their rehabilitation goals
- Collaborate with the therapy team to ensure seamless care and communication
- Document resident progress and treatment plans in a timely and accurate manner
- Graduate of an accredited Physical Therapy program
- Current New Jersey Physical Therapy License
- Experience in long-term care settings preferred
