Job Summary

We are seeking a skilled Nurse Care Coordinator to join our team at Mercy Care Atlanta. This is a full-time position that requires day shift availability.

About the Role

This role involves coordinating access to healthcare, health education, and preventive services for our diverse patient population. As a Licensed Practical Nurse (LPN), you will work under the supervision of a Mercy Care Medical doctor and/or Nurse Practitioner to coordinate access to healthcare, provide patient care, and support preventive services.

Key Responsibilities:

  • Coordinate access to healthcare, health education, and preventive services for target population.
  • Perform basic patient assessment procedures including vital signs, infection disease control, and universal precautions.
  • Maintain and order medical supplies and materials under the guidance of Nurse Practitioners and/or Clinic Manager.
  • Perform EKGs under the guidance of a licensed Medical Provider.
  • Participate in health fairs or community events on weekends as requested.

Minimum Qualifications:

  • Graduate of an accredited school of Practical Nursing.
  • Current Georgia State license.
  • One-year experience in an ambulatory care setting preferred; new graduates are welcome to apply.
  • Passing score on entry medication exam.
  • Current CPR or BCLS certification.
  • Knowledge of basic arrhythmia interpretation within one year if hired for a telemetry unit.

Estimated Salary: $45,000 - $65,000 per annum.
