About the Job:
As a Patient Care Coordinator, you will be responsible for reviewing patient charts, identifying areas for improvement, and implementing changes to enhance patient care. You will also develop and implement patient care plans, collaborate with healthcare providers to resolve issues, and monitor patient progress. Your exceptional communication and interpersonal skills will enable you to build strong relationships with patients, families, and healthcare providers, ensuring effective coordination of care. Additionally, you will maintain accurate records, report any concerns or issues, and participate in quality improvement initiatives.

Requirements:
To be considered for this position, you must possess a valid Georgia driver's license, a state of Georgia license as a Licensed Practical Nurse, and a minimum of two years of experience as a Licensed Practical Nurse. You must also be able to demonstrate proficiency in performance of the essential functions and learn, comprehend, and apply all county or departmental policies, practices, and procedures necessary to function effectively in the position.

Estimated Salary: $48,000 - $65,000 per year
