RN (Per Diem/On Call) - PACE/Elderplace

We are seeking a skilled Registered Nurse to join our PACE/Elderplace team in a per diem/on call capacity. As a key member of our interdisciplinary team, you will provide care management, care coordination, and skilled care services to at-risk frail elder patients with complex medical and psychosocial issues.
Key Responsibilities- Provide care management and care coordination services to patients with complex medical and psychosocial needs
- Develop and implement individualized care plans to improve patient outcomes and quality of life
- Collaborate with the interdisciplinary team to ensure seamless care transitions and coordination
- Provide skilled care services, including wound care, medication management, and vital sign monitoring
- Assist providers in identifying patient needs and coordinating access to appropriate care and services
- Current Oregon Registered Nurse License
- National Provider BLS - American Heart Association certification within 30 days of hire
- Relevant certifications applicable to the position, as specified by the Nurse Manager
- Bachelor's Degree in Nursing
- 1 year of home health RN experience
