**Responsibilities:**

  • Provide direct patient care to residents in a long-term care setting
  • Collaborate with the interdisciplinary team to develop and implement individualized care plans
  • Participate in quality improvement initiatives and contribute to a culture of safety and excellence
  • Communicate effectively with residents, families, and healthcare team members

**Requirements:**

  • Current license to practice as a Licensed Practical Nurse (LPN) or Registered Nurse (RN) in the State of New York
  • Ability to work a rotating shift schedule, including weekends and holidays
  • Strong communication and interpersonal skills
  • Ability to work effectively in a team environment
