Salary Information$62,000 - $70,000 per year, including an estimated bonus based on productivity.

Job DescriptionAs a Support Coordinator Intellectual Disabilities, you will be responsible for providing supports coordination services to individuals diagnosed with intellectual disabilities. This includes assessing needs, developing annual plans, locating appropriate services, coordinating services and supports, monitoring services, and referring to community and system resources.

Key Responsibilities
  • Provide supports coordination services, including assessment of needs, annual plan development, service location, coordination, monitoring, and referral.
  • Meet face-to-face with individuals and families to write an annual plan according to regulations and guidelines.
  • Work with individuals, families, other professionals, and community members to identify needs and desires.
  • Act as an advocate for individuals in the community and with public and private entities.
  • Assist individuals/families in obtaining financial, transportation, recreation, respite, adaptive equipment, home adaptation services, behavioral health, medical resources, and other supports as needed.
