Nurse Practitioner Supervisor Job Description

Job Summary:

The Nurse Practitioner Supervisor will work in collaboration with community primary care physicians to ensure proper oversight and coordination of participant care with the Interdisciplinary Team.

This role will work as the liaison between the participant, the community physician, and plan staff to develop and implement the plan of care, including person-centered goal development, communicating changes in participant condition, and facilitating primary care preventative services.

Key Responsibilities:

  • Supervise and manage Nurse Practitioners assigned to their location and assist with the oversight of other Diagnostic Treatment Center's clinical staff as needed.
  • Complete direct reports annual performance appraisals and collaborate with other DTC Managers providing annual performance feedback of the DTC clinical staff as needed.
  • In the absence of, or at the direction of, the Medical Director, the NPS will represent or serve as the Medical Director role at their location.
  • Assess and manage participants with acute changes in condition in a timely manner.
  • Provide urgent care services to participants and facilitate the provision of same day access.
  • Assess participants upon return from the hospital/Emergency Department within 5 days and reconcile medications and treatment plans.
  • Review patient's past medical history at least every 6 months and formulate a comprehensive and complete diagnostic list of current and past medical conditions.
  • Participate in all clinical documentation improvement activities to ensure accurate and full capture of the burden of illness of our participants.
  • Provide feedback and counseling to DTC and IDT staff regarding their clinical documentation as needed.
  • Communicate with CL's IDT team or other CL Plan designees in accordance with CL's policies and procedures.
  • Participate in complex care management and educate participants and their families on disease processes, medication adherence, and self-management promotion.
  • Comply with all HIPPA regulations and maintain security of Protected Health Information.

Requirements:

  • Graduated from a nurse practitioner education program acceptable to NYSED or certified as a nurse practitioner by a national certifying organization acceptable to NYSED.
  • Minimum of two years in clinical nursing practice in-home care, care management, or related field required.
  • Effective oral and written communication and interpersonal skills required.
  • High-level analytical and research skills.
  • Ability to manage multiple projects and meet project deadlines.
  • Basic Computer Skills in Microsoft office.
  • Licensed to practice as a Nurse Practitioner in NY required.
  • DEA Certification required.
  • AANP or ANCC Board Certification required.
