About the Opportunity:

Schedule:
Monday to Friday, 8:00am to 5:00pm, with a 1-hour break

Responsibilities:

The successful candidate will:
Oversee and manage a team of Nurse Practitioners, ensuring seamless clinical operations and providing guidance as needed.
Conduct annual performance evaluations and provide constructive feedback to team members.
Perform comprehensive wellness assessments for participants, taking into account their complex medical histories and comorbidities.
Provide urgent care services and facilitate same-day access to healthcare services.
Assess participants upon return from hospital or emergency department visits within 5 days.
Review patient medical histories every 6 months and document comprehensive diagnostic lists.
Participate in clinical documentation improvement activities to ensure accurate and complete patient records.
Review participants' current symptoms and exacerbations of previously controlled conditions.
Perform other duties as required.

Qualifications:
At least 2 years of clinical nursing experience in a care management, home care, or related setting.
Graduate of an accredited Nurse Practitioner program.
NYS Nurse Practitioner license.
AANP or ANCC Board Certification.
DEA certification.
Computer proficiency and Microsoft Office skills.

Desired Skills:
Bilingualism in English and Spanish.
