Clinical Nurse Specialist

We are seeking a skilled and compassionate Triage Nurse to join our interdisciplinary team at LHC Group. As a key member of our hospice care team, you will be responsible for delivering high-quality, patient-focused care through triage calls after hours and on holidays.
Responsibilities- Receive and respond to calls from patients and/or family members, assessing problems and delivering compassionate care.
- Document calls received within the electronic medical records system, ensuring timely and accurate processing of workflow.
- Coordinate with agency on-call nurses to deliver high-quality nursing care and schedule assessments as needed.
- Maintain effective communication with patients, families, employees, physicians, vendors, and potential referral sources.
- Update staff and interdisciplinary team members regarding patient changes, vendor and/or facility changes to ensure compliance with the patient's plan of care.
- Assist in overseeing the utilization and ordering of medications, DME, and supplies related to the plan of care.
- Ensure appropriate education regarding updates, processes, and regulations, attending mandatory educational offerings and in-services.
- Licensed Registered Nurse in good standing with no restrictions in all states of practice.
- Two years' experience as a Registered Nurse with at least one year of direct patient care in a hospice, home health, or oncology setting.
