We are seeking a highly skilled and compassionate Registered Nurse (RN) to join our team as an MDS Coordinator at American Senior Communities. As a key member of our healthcare team, you will play a vital role in promoting individualized support and care of our residents.
Key Responsibilities- Coordinate the completion of assessments using RAI tools to ensure accurate and comprehensive care plans.
- Act as a resident advocate by managing the interdisciplinary care plan process, ensuring that each resident receives personalized attention and support.
- Oversee the management of resident assessments for case mix reimbursement and transmit MDS reports according to strict guidelines.
- Participate in Quality Improvement committees to enhance positive resident outcomes and contribute to the development of best practices in long-term care.
- Demonstrate C.A.R.E. values in all interactions with residents, family members, customers, and staff.
- Graduate of an accredited School of Nursing, preferably with a Bachelor of Science in Nursing (BSN) degree.
- Minimum of one year of experience in nursing management in the long-term care industry.
- Two years of professional nursing experience in long-term care, acute care, restorative care, or geriatric nursing settings.
- Knowledge of the Resident Assessment Instrument, Interdisciplinary Care planning process, and Minimum Data Set 2.0.
- Knowledge of OBRA regulations and familiarity with current state reimbursement guidelines.
- Knowledge of current laws and regulations that influence the provision of care and services in nursing facilities.
