VIVA Pediatrics is a dedicated provider of home health services, specializing in private duty nursing, therapy, and skilled nursing visits tailored for infants and children. Our mission is to deliver an outstanding level of care to each patient.
Key Responsibilities:- Conduct comprehensive assessments of home health patients to determine their physical, psycho-social, and environmental needs, as documented through clinical records and evaluations.
- Develop, implement, and document individualized care plans to ensure the highest quality and continuity of care.
- Administer care while adhering to infection control protocols that safeguard both staff and patients, in compliance with OSHA standards.
- Follow all VIVA Pediatrics policies and procedures, promoting the agency's philosophy.
- Facilitate effective communication with patients, families, team members, and other healthcare professionals, as evidenced by clinical documentation and evaluations.
- Promptly inform the supervising nurse of any changes in the patient's condition.
- Provide health education to clients and/or caregivers to enhance understanding and compliance.
- Timely document any interim changes to the care plan.
- A minimum of 1 year of experience or 6 months in pediatric home health as a licensed RN or LVN.
- Current CPR Certification.
- Valid Driver's License.
