Job Description

We are seeking a skilled RN Care Manager to join our team at Good Samaritan Hospice's Roanoke location. As an RN Care Manager, you will be responsible for managing patient care and providing services that require substantial, specialized bedside nursing skills for symptom management, crisis intervention, and family education.

Responsibilities

  • Develop and maintain rapport with patients and families
  • Work effectively with an interdisciplinary team
  • Travel within the Good Samaritan Hospice service area, including overnight travel outside of the area for continuing education
  • Be at direct risk for exposure to infections and blood-borne pathogens

Requirements

  • Graduate from an accredited school of nursing
  • Currently licensed to practice nursing by the Virginia State Board of Nursing
  • Two years' experience in acute care, home health, hospice, med/surg, or geriatric nursing
  • Current Virginia driver's license and driving record that meets the standards set forth in current policies
  • BLS (AHA)
  • Virginia state license

Salary Information

The estimated salary for this position is $2200 per week.
