Job Summary

We are seeking a skilled and compassionate Care Manager to join our team at Medi Home Health & Hospice. As a key member of our healthcare team, you will provide high-quality nursing care to patients in their homes, promoting independence and well-being.

About the Role:

  • Provide comprehensive nursing assessments and develop individualized plans of care for patients with complex needs.
  • Collaborate with interdisciplinary teams, including physicians, therapists, and social workers, to ensure seamless care coordination.
  • Initiate and implement patient education programs, focusing on disease management, medication adherence, and healthy lifestyle practices.
  • Maintain accurate and detailed records of patient interactions, progress, and outcomes, using electronic health records (EHRs).
  • Evaluate patient response to treatment and make necessary adjustments to care plans.
  • Provide emotional support and counseling to patients and their families, addressing concerns and fears related to illness or disability.

Requirements and Qualifications:

  • A minimum of one year of experience as a Registered Nurse (RN) in home healthcare or a related setting.
  • Current licensure as an RN in Virginia.
  • Valid driver's license and reliable transportation.
  • Strong communication and interpersonal skills, with the ability to work effectively with diverse populations.
  • Proficiency in electronic health records (EHRs) and other healthcare software applications.
