Seasoned Recruitment is looking for a dedicated and empathetic Nurse Practitioner specializing in Primary Care to become a vital part of our interdisciplinary team. This role focuses on delivering comprehensive medical services to participants in the PACE (Program of All-Inclusive Care for the Elderly) program.
Key Responsibilities:
- Deliver direct medical services to PACE participants.
- Conduct assessments and contribute to care planning.
- Oversee a designated group of participants.
- Initiate, assess, and modify medication and treatment orders with necessary approvals.
- Coordinate daily care and evaluations under supervision.
- Maintain accurate documentation of participant interactions and assessments in the EMR system within required timeframes.
- Facilitate referrals to specialists as needed.
- Perform comprehensive history and physical examinations and provide participant education.
- Order and interpret laboratory tests and manage follow-ups.
- Execute specific medical procedures as required.
- Identify and escalate any abnormalities promptly.
- Collaborate with team members for holistic participant care, including behavioral health and pharmacy.
- Evaluate and provide care to participants in various settings, including home and virtual environments.
- Engage in advanced care planning discussions with participants.
- Document chronic conditions for HCC capture.
Schedule and Work Details:
This position offers a 40-hour work week with no on-call requirements. Schedules may vary.
