Primary Care Nurse Practitioner

We are seeking a highly skilled and compassionate Nurse Practitioner to join our team at InnovAge. As a key member of our interdisciplinary team, you will provide primary care to our enrolled participants, evaluating their physical complaints and providing appropriate treatment.
Key Responsibilities- Evaluate participants and document information in their medical records.
- Perform physical examinations, order and interpret diagnostic tests, and provide treatment plans.
- Manage acute and long-term medical needs of participants in collaboration with other primary care providers.
- Provide emotional and spiritual support to participants, as well as education on healthcare issues and treatments.
- Manage medical emergencies as needed and ensure timely documentation of medical care in participant records.
- Current State-issued Nurse Practitioner license.
- 1+ year of experience as a practicing Nurse Practitioner.
- DEA Certification with full prescriptive authority.
- 5+ years of experience as a Registered Nurse with experience in elderly care.
