Key Responsibilities
  • Perform comprehensive assessments and screenings in an in-home setting, including annual wellness visits and quality gap care visits.
  • Accurately code, verify, and document clinical findings, ensuring seamless communication with our team and members.
  • Utilize our proprietary tool or client EMR to complete detailed evaluations of medical and social assessments, identifying areas for referral and follow-up.
  • Develop and implement personalized care plans, collaborating with members and their primary care physicians to ensure optimal health outcomes.
  • Maintain accurate and up-to-date records, adhering to HIPAA guidelines and ensuring confidentiality.
  • Provide exceptional customer service, fostering strong relationships with members and their families.
Requirements
  • Current, unrestricted license in the state(s) in which the services are rendered.
  • Board-certified, preferably in geriatric or adult medicine specialty.
  • 2 years of hands-on experience as a Nurse Practitioner, with a strong background in in-home care.
  • Familiarity with the HCC model and quality HEDIS and other STAR measures.
  • Excellent communication and interpersonal skills, with the ability to effectively engage with elderly and chronically ill members.
  • Proficient in the use of computer, tablet, and mobile technology.
