We are seeking an Advanced Practice Provider to join our innovative team at Trellis Health, a subsidiary of MaineHealth. As a key member of our care team, you will play a crucial role in shaping our clinical practices and patient experiences from the ground up.

Responsibilities
  • Manage a comprehensive patient panel, fostering trust-based relationships and proactive healthcare management.
  • Collaborate closely with other team members to develop, update, and prioritize patient care plans.
  • Ensure team-driven adherence to these plans, focusing on holistic health outcomes, patient satisfaction, and effective resource utilization.
Requirements
  • Education: Successful completion of an approved Nurse Practitioner or Physician Assistant program.
  • License/Certifications: Current nursing license issued by the State of Maine or eligible for licensure. Current BLS certification.
  • Experience: Minimum three years of professional nursing experience. Two years of Nurse Practitioner or Physician Assistant experience preferred.
  • Strong communication, interpersonal, and customer service skills.
  • Ability to maintain confidentiality and work effectively in a virtual setting.
