As a key member of our Adult Residential Treatment Team, you will play a vital role in providing high-quality care to individuals living with serious and persistent mental illness, substance misuse, and homelessness.
Job Summary:The Residential Nurse or Licensed Practical Nurse will be responsible for evaluating medication management practices, providing direct care to clients, and coordinating physical health needs with medical providers. You will work closely with the Director to achieve positive program and client outcomes, utilizing a recovery-oriented, trauma-informed, and person-centered approach to care.
Responsibilities:- Providing direct nursing care, within the scope of RN practice, and as medically ordered, for clients in the residential program;
- Support integrated physical health care, assist with admission and discharge, providing care transition planning and care coordination and monitor medication procedures;
- Contributing to the achievement of positive client outcomes and results by working closely with Residential Director and developing wellness programming;
- Providing training, teaching, coaching, instruction, consultation and health education for residential staff that contributes to the achievement of positive client outcomes;
- Licensed practical nurse holding a valid license to practice nursing in the State of Maine, and license must be in good standing are required; with two (2) to four (4) years of experience required, with recommendation that one of those years being in clinical practice in mental health nursing, or Registered Nurse holding a valid license to practice nursing in the State of Maine, and license must be in good standing are required; with two (2) to four (4) years of experience required, with one of those years being in clinical practice in mental health nursing;
- Previous training experience in care coordination and/or integrated care is preferred;
- Experience within the Mental Health and Substance Use delivery system and/or direct service is preferred;
- Experience in the education and clinical care of clients with diabetes is a plus;
- Advanced computer skills in programs including Microsoft Word, Excel, and PowerPoint required;
- Strong written and oral communication and interpersonal skills required;
- Strong attention to detail required;
- Experience in team or multidisciplinary approach to service planning is preferred;
