Job Summary

The Enhanced Care Management Coordinator will oversee and coordinate care management services for individuals with complex medical and behavioral health needs. This role involves working closely with healthcare providers, insurance companies, and other members of the care team to ensure the delivery of high-quality, comprehensive care.

Responsibilities
  • Coordinate and monitor the delivery of healthcare services, ensuring adherence to care plans and quality standards.
  • Facilitate communication and collaboration between healthcare providers, insurance companies, and other members of the healthcare team.
  • Provide education and support to individuals and their families regarding their care plans and available resources.
  • Stay updated on industry best practices and guidelines related to care management.
  • Collaborate with community organizations and agencies to enhance care coordination services.
  • Participate in ongoing quality improvement initiatives to ensure the delivery of high-quality care.
Qualifications and Education Requirements
  • Bachelor's degree, preferably in health sciences.
  • Bilingual: English and Spanish preferred.
Experience

Minimum of 0-1 years of coordinator experience in healthcare/health services, with at least 1 year of experience in Medi-Cal managed care and/or as a Medical Assistant.

Experience working with vulnerable populations such as individuals experiencing homelessness, chronic medical conditions, substance use and/or mental health conditions and/or residing in long-term care is preferred but not required.

  • Understanding of and sensitivity to mental health conditions, substance use disorders, and multi-cultural communities.
Physical Demands

May be required to lift objects weighing up to 25 pounds. Sitting in normal seated position for extended periods. Reaching by extending hand(s) or arm(s) in any direction. Finger dexterity is required to manipulate objects with fingers rather than whole hand(s) or arm(s), for example using a keyboard.
