Key Responsibilities
  • Coordinate and implement the TOC/DC plan for ambulatory patients
  • Prioritize and coordinate the plan across the care continuum through critical thinking, teamwork, and communication between care providers, patients, families, and external vendors
  • Ensure timely discharge and coordinate with care providers to ensure a smooth transition
  • Develop and maintain effective relationships with patients, families, and care providers
  • Stay up-to-date with changing regulations and guidelines related to care transitions
Requirements
  • Associate's degree in Business or a related field
  • 1 year of recent relevant experience
  • Excellent oral and written communication skills
  • Strong interpersonal and time management skills
  • Ability to work effectively in a fast-paced environment with rapidly shifting priorities and competing demands
  • Intermediate PC skills and word processing skills
Working Conditions

This is a full-time position with a regular schedule of 40 hours per week. The pay range for this role is $24.15 to $32.60 per hour.
