Job Title: Social Service Assistant

Join Axesspointe Community Health Center as a Social Service Assistant and contribute to providing a safe and stimulating environment for residents.

Job Summary:

The Social Service Assistant will work in conjunction with the Senior Support staff to provide exceptional support services to residents. This role requires strong communication and interpersonal skills, as well as the ability to work independently and as part of a team.

Key Responsibilities:
  • Assist in the efficient running of the facility and its therapeutic programming.
  • Answer phones, direct inbound calls, and take messages.
  • Keep the front office organized and maintain accurate records.
  • Prepare rooms for incoming residents and ensure a smooth transition.
  • Coordinate with vendors for facility needs and maintain accurate logs.
  • Process donations and complete check requests as needed.
  • Prepare meals for residents on occasion and track therapeutic attendance.
  • Complete contact summaries and address potential client inquiries.
  • Assist residents with communication and liaison with various agencies.
  • Accompany residents to appointments as needed and promote anti-discriminatory practices.
  • Adapt to the environment and provide support to ensure residents experience an improved quality of life.
  • Attend staff meetings and trainings as required and foster positive working relationships with staff members.
  • Maintain up-to-date knowledge of internal fire regulations and procedures.
  • Report timely for all assigned shifts and comply with agency policies and procedures.
Qualifications:
  • Ability to model open communication, fair and equitable treatment, and display caring and compassion.
  • Excellent communication skills for team members and clients, even in stressful situations.
  • Knowledge of community services and Microsoft Word skills.
  • Valid Ohio Driver's License and ability to meet agency requirements for driving insurability.
  • Ability to meet agency requirements for criminal background check and have reliable transportation and a phone.
Preferred Qualifications:
  • CPR/First Aid Certification.
  • Experience working in crisis intervention.
