About the Role

Amergis is seeking a skilled Licensed Clinical Social Worker to join our team. As a key member of our multi-disciplinary team, you will provide critical social work intervention and counseling services to patients and clients in a variety of settings.

Key Responsibilities
  • Collaborate with the team to develop and implement plans of care for patients requiring social work intervention.
  • Provide counseling and crisis intervention services to individuals and families in need.
  • Assess patient psychosocial needs and develop interventions to meet established goals.
  • Collaborate with all disciplines to plan and evaluate team goals for each patient.
  • Facilitate referrals to community agencies and provide financial counseling as needed.
  • Respond to crisis situations, including domestic violence and child abuse.
  • Provide witness services and evidence in court cases.
  • Visit schools to assess academic concerns and work with government agencies to help clients apply for Medicare and Medicaid.
  • Arrange home visits to evaluate patient health, home environment, and family interaction.
  • Assist patients and families in planning for present and future psychosocial needs.
  • Report patient progress and changes in patient status to the physician and team members.
Requirements
  • Master's degree in Social Work from a Council on Social Work Education accredited school.
  • Active license as an LCSW in the state of practice.
  • Minimum of one year of social work experience preferred.
  • Compliance with all relevant professional standards of practice.
  • Participation and completion of Amergis' Competency program when applicable.
  • Current CPR and TB questionnaire, PPD, or chest x-ray if applicable.
  • Current Health certificate (per contract or state regulation).
  • Must meet all federal, state, and local requirements.
  • Successful completion of new hire training as applicable to the job site.
  • Understanding of patient confidentiality and HIPAA requirements.
  • Ability to effectively elicit and provide information to and from appropriate individuals via strong communication skills.
  • Computer proficiency required.
  • Must be at least 18 years of age.
