Hospice Community Liaison RN

A dynamic position that plays a crucial role in accepting referrals for Hospice, Home Health, and Home Infusion services in various settings, including acute care, physician clinics, and select nursing homes and rehabilitation centers in the St. Louis and Washington areas.
Maintaining an active presence in these settings, this role delivers exceptional customer service to discharge planners, medical social workers, physicians, and future patients. The successful candidate will facilitate timely discharges and support the system's initiative for continuity of care within the Mercy Health System.
Responsibilities- Accept referrals for Hospice, Home Health, and Home Infusion services in various settings.
- Maintain an active presence in these settings to deliver exceptional customer service.
- Facilitate timely discharges and support the system's initiative for continuity of care.
- Registered Nurse (RN) license in Missouri.
- Minimum three years of nursing experience, preferably in Hospice and Home Health.
- Excellent observation, nursing assessment, and communication skills.
- Ability to provide written documentation in a timely manner.
- Self-directed with the ability to work with little supervision.
- Experience in providing Hospice and Home Health nursing care.
- Certification in Hospice and Palliative Nursing (CHPN).
- Basic Computer Skills and understanding of Electronic Medical Records.
