About the Role

Gentiva is seeking a compassionate and experienced RN Clinical Liaison to join our team. As a key member of our hospice care team, you will play a vital role in educating healthcare professionals about our services and providing end-of-life solutions with clinical excellence, compassion, and dignity.

Key Responsibilities
  • Ensure effective and timely intake and referral processes to meet patient, family, and referral source needs
  • Complete admissions processes for emergency cases and inpatient hospice admissions
  • Provide accurate and timely information about hospice services to inquirers
  • Respond to referral source calls, initiate paperwork, and begin the intake/referral process
  • Serve as a patient advocate in evaluating and developing plans of care and obtaining informed consent for hospice
  • Build relationships with multiple account types, including SNFs, ALFs, hospitals, and physicians
  • Offer in-services and education on the benefits of hospice
Requirements
  • Hospice heart: compassionate, kind, and empathetic to clients, patients, and families
  • RN license in the state(s) of employment
  • Minimum 2 years of healthcare experience in hospice, home health, clinical liaison, RN liaison, clinical coordination, discharge planner, hospice case manager, or RN educator
  • Known and respected within your community
