Clinical Liaison RN Inpatient Hospice Job Description

**Overview**

AccentCare is seeking a compassionate and experienced Clinical Liaison RN to join our team in Inpatient Hospice. As a Clinical Liaison RN, you will play a vital role in providing exceptional patient care and support to our patients and their families.

**Responsibilities**

* Develop and implement care plans that meet the unique needs of each patient
* Collaborate with healthcare teams to ensure seamless transitions of care
* Provide education and support to patients and families on hospice care and services
* Conduct assessments and evaluations to identify patient needs and develop individualized care plans
* Maintain accurate and up-to-date patient records

**Requirements**

* One year of RN experience in local market with a strong knowledge of governmental home health regulations, Medicare requirements, and care plan development techniques
* Previous medical sales experience preferred
* Current Washington DC RN License
* Must be a licensed driver with an automobile that is insured and in accordance with state and/or organization requirements and in good working order
