Position Summary: In the role of Health Navigator, you will be instrumental in assisting patients as they navigate the complexities of the healthcare system. Your key responsibilities will encompass building trusting relationships with patients, managing patient-related tasks, coordinating care with healthcare professionals, and providing education on available healthcare options. Additionally, you will monitor patient progress, arrange appointments, and connect patients with essential community resources.

  • Establish and nurture strong relationships with patients to support their healthcare needs
  • Assist patients in adhering to prescribed medications and participating in preventive health screenings
  • Conduct follow-ups on patient requirements and organize clinic visits
  • Facilitate communication with healthcare providers through electronic health records
  • Support administrative functions related to patient outcomes
