Responsibilities
  • Perform comprehensive patient assessments and develop individualized care plans to meet the unique needs of each patient.
  • Collaborate with our multidisciplinary team of healthcare professionals to ensure seamless care coordination and achieve optimal patient outcomes.
  • Supervise and mentor LPNs and Home Health Aides to ensure the highest level of care and support for our patients.
  • Document patient information accurately and efficiently using our electronic health record system.
Requirements
  • Active RN license in the state of service.
  • At least one year of clinical experience in home health or a related field.
  • CPR certification.
  • Valid driver's license.
