RN Care Manager

We are seeking a highly skilled RN Case Manager to provide exceptional care to our patients and ensure they receive the necessary resources and support to achieve optimal health outcomes.
Key Responsibilities- Assess and develop care plans for patients in collaboration with physicians and other healthcare providers
- Monitor and evaluate patient progress and adjust care plans as needed
- Provide education and support to patients and their families on disease management, medications, and treatment options
- Coordinate patient care and communicate with providers, payers, and other stakeholders as needed
- Advocate for patients and ensure they receive appropriate care and resources
- Document patient care and maintain accurate records
- Participate in quality improvement initiatives and ensure compliance with regulatory requirements
- Active RN license with minimum of 2 years of clinical experience
- Strong clinical assessment and critical thinking skills
- Excellent communication and interpersonal skills
- Ability to work independently and as part of a team
- Knowledge of healthcare systems, regulations, and reimbursement processes
- Proficiency in electronic health records and other healthcare technology
