Long-Term Services and Supports Specialist

The Long-Term Services and Supports (LTSS) Specialist at CareBridge collaborates with Managed Care Organizations to assist individuals aging in place who are receiving home and community-based services. This role involves conducting telephonic and/or virtual evaluations to ascertain participants' needs, taking into account their health history, functional capabilities, environmental safety, and social support systems. The LTSS Specialist provides informed recommendations to MCOs regarding the types and hours of supportive services necessary.
Key Responsibilities:- Perform objective evaluations for program participants to establish the appropriate level of support and services required.
- Gather comprehensive and accurate participant histories to inform the overall assessment process.
- Conduct timely reassessments annually or more frequently as dictated by program requirements and participant needs.
- Engage with program participants and MCO representatives to provide recommendations based on assessed needs.
- Educate participants and MCO representatives on available options for home modifications, durable medical equipment (DME), assistive technology, and other adaptive tools.
- Support the Clinical team with recommendations for necessary equipment and services.
- Work collaboratively with the Decision Support team to report observations and outcomes.
- Document all client interactions in accordance with established documentation standards.
- Maintain a current understanding of home and community-based services, MCO processes, and relevant state regulations.
- Achieve or surpass productivity standards and performance benchmarks.
- Exhibit excellent punctuality and attendance during scheduled work hours.
- Perform additional duties as assigned.
Required:
- Valid OTA, PTA, OT, or PT license without restrictions.
- Minimum of 2 years of experience in social services or a healthcare-related field.
- Experience working with individuals with disabilities and seniors.
- Familiarity with the home and community-based service system.
- Ability to review and interpret medical histories proficiently.
- Experience in conducting standardized assessments.
- Willingness to engage in phone communication for 50% - 60% of the workday.
- Cultural competence and professionalism in interactions with program participants, MCO representatives, and interdisciplinary team members.
- Strong written and verbal communication skills, capable of conveying information to individuals of diverse ages and abilities.
- Team-oriented with a commitment to building strong working relationships within the organization.
- Technical proficiency with customer service software, call center management tools, Windows, and Office applications.
Preferred:
- Experience with home assessments, CAPS, or ATP certifications.
- Background in home health services.
