Hospice Nurse Case Manager

As a Hospice RN Case Manager, you will play a vital role in providing high-quality care to patients and their families, ensuring their comfort, dignity, and well-being throughout the end-of-life journey.
Job SummaryWe are looking for a dedicated and empathetic Hospice RN Case Manager to join our team. As a Hospice RN Case Manager, you will be responsible for assessing patient needs, developing and implementing individualized care plans, and coordinating care with the interdisciplinary team. You will also provide emotional support and education to patients and their families, ensuring they receive the best possible care.
Responsibilities- Conduct comprehensive assessments of patients and their families to determine their needs and develop individualized care plans.
- Coordinate care with the interdisciplinary team, including physicians, social workers, and other healthcare professionals.
- Provide emotional support and education to patients and their families, ensuring they receive the best possible care.
- Manage patient care plans, including medication administration, wound care, and other treatments.
- Collaborate with the hospice team to ensure seamless care transitions and effective communication.
- Bachelor of Science in Nursing (BSN) and current RN license.
- Minimum of two years' experience as a professional nurse, with at least one year in hospice or home care.
- Excellent communication and interpersonal skills, with the ability to work effectively with patients, families, and healthcare teams.
- Ability to work independently and as part of a team, with a strong commitment to patient-centered care.
