We are seeking a skilled Licensed Practical Nurse (LPN) to join our team as a Home Care Transition Specialist. This is an exciting opportunity to work in a dynamic and supportive environment, providing high-quality care to patients in their homes.

About the Role

As a Home Care Transition Specialist, you will play a crucial role in supporting the transition of care from hospital or healthcare facility settings to home care services. Your expertise will enable you to deliver exceptional patient care, working closely with our multidisciplinary team to ensure seamless transitions.

Key Responsibilities
  • Deliver high-quality patient care in accordance with established standards and protocols.
  • Conduct thorough assessments of patient needs and develop individualized care plans.
  • Collaborate with healthcare teams to ensure effective communication and coordination of care.
  • Maintain accurate records and documentation of patient interactions and care provided.
Requirements

To be successful in this role, you will need:

  • A current license as a Licensed Practical Nurse (LPN).
  • NICU, PICU, ICU, or home care trach/vent experience required.
  • Achieve a passing score on high-tech Adult and Pediatric simulations.
  • Hold current CPR certification.
  • Ability to work in a teaching setting to prepare and educate the ongoing BAYADA care team.
