Job Summary

Naven Health is seeking a highly skilled and compassionate Home Infusion Nurse to join our team. As a Home Infusion Nurse, you will play a critical role in delivering exceptional care to patients in the comfort of their own homes.

Key Responsibilities
  • Coordinate Patient Care: Develop and implement individualized plans of care for patients, ensuring continuity of care and effective communication with the healthcare team.
  • Provide Direct Patient Care: Administer medications, perform treatments, and educate patients and families on proper care and management of their conditions.
  • Assess Patient Needs: Conduct thorough assessments of patients' physical and emotional needs, identifying areas for improvement and developing strategies to address them.
  • Communicate Effectively: Maintain open and transparent communication with patients, families, and healthcare teams, ensuring seamless coordination of care.
  • Document Patient Care: Accurately and thoroughly document patient care, including progress notes, medication administration records, and other relevant information.
  • Collaborate with Healthcare Teams: Work closely with physicians, pharmacists, and other healthcare professionals to ensure comprehensive and coordinated care.
  • Stay Up-to-Date with Industry Developments: Participate in ongoing education and training to stay current with the latest advancements in home infusion therapy and patient care.
Requirements
  • Education and Experience: Graduate of an accredited School of Nursing with at least 2 years of clinical experience working as a RN in an area requiring training and hands-on experience in infusion therapies.
  • Certifications and Licenses: Current active and unrestricted registered nurse license, Basic Life Support for Health Care Professionals certification, and IV nursing certification (if applicable).
  • Skills and Abilities: Demonstrated competency in and knowledge of INS standards, vascular access skills, and ability to communicate effectively with patients, families, and healthcare teams.
